Salary: up to £43,000 per annum

Hours: 37.5 hours

Job Type: Permanent – Full time

Location: Hybrid – Balham / Ealing & Home working

About the role

The role of a Referral and Assessment Lead is to manage referrals to Certitude from people seeking support or from statutory authorities wishing to commission services from Certitude. The role sits within the Housing team and is critical in supporting timely, effective, quality referrals and assessments of people into Certitude homes.

Duties will include:

  • Oversee the local processes and procedures for local mental health contracts outside the referral procedure ensuring all empty rooms are relet with minimum delays.
  • Work closely with social workers, and other stakeholders to gather relevant information and make informed decisions.
  • Partner with Operational, Finance and Housing colleagues to monitor and plan the re-let of properties according to individual and organisational needs and in compliance with existing nominations and re-let agreements.
  • Be the main organisational point of contact for all new referrals - Learning disabilities and mental health.
  • Keep, analyse and report statistical data, as required including taking responsibility for the single source of truth weekly empty rooms report ensuring circulation to all internal stakeholders.

About you

To be a Referral and Assessment Lead at Certitude, the following are essential:

  • Extensive experience in referrals, assessments and placement management, with proven expertise in assessing people’s needs
  • Expert knowledge of Referral and Assessment processes within adult social care and accommodation-based support
  • Strong knowledge of Care Standards and Care Act 2014, safeguarding, DOLS/MCA and CQC regulatory framework
  • Strong background in Adult Social Care, with knowledge of learning disabilities, autism and mental health
